to take an active role in self managing their care. One way to achieve this is to provide patients
with practical and relevant information. Princess Margaret has developed the Cancer
Journey resources for all new cancer patients at Princess Margaret Cancer Centre. What are the Cancer Journey Resources? The Cancer Journey Resources are meant to
be given to all new cancer patients. They contain information to help patients
throughout their cancer journey. The Cancer Journey Resources are meant to
augment your clinical teaching, and empower patients to self manage their own care. The print version of the Cancer Journey Resources
comes in two parts. The first is as portable planner that includes
interactive sheets for patients to use. A calendar to keep track of upcoming appointments. A section for appointments and any question
they may have for you. A medicine tracker that can help patients
remember when and how much of a medicine or supplement they should be taking. And a side effects tracker, where they can
write down how they have been feeling and receive coping methods from you or another
member of their health care team. Using a planner can help patients to be more
prepared for appointments, by having their questions ready and noting how they are feeling. The second Cancer Journey Resource is a reference
Guide Book that patients can keep at home. The Guide Book provides practical tips and
information and a descriptive list of resources at Princess Margaret Cancer Centre and in
the community. There is a pocket in the back of the Guide
Book where patients can keep pamphlets or documents they are given. How do patients get the Cancer Journey Resources? The Cancer Journey Resources are available
in print format, online, or through the Cancer Journey App. New patients can get a print copy of the Cancer
Journey Resources from you when they come for their new patient visit. During new patient appointments introduce
the Cancer Journey Resources to patients and their families. When you introduce the Cancer Journey Resources,
it may be a good time to acknowledge that you understand that patients get a lot of
information all at once, and it can be overwhelming. You can tell patients that health care providers
at Princess Margaret Cancer Centre made the guide book to give patients practical tips
and information they can read through at their own pace. Let patients know to bring the planner with
them to appointments to take notes. Tell them they can also write down questions
they want to ask, and show you their side effects tracker. Let patients know that they can take a print
